If you live outside Canada, please print the form, fill it out and send the completed form to the Ministère by email, regular mail or fax.

To obtain a duplicate transcript for the English Test (1992 to 1997), please print the form, fill it out and return the completed form to the Ministère by email, regular mail or fax.
